AccessCare Southern Home and Community Care Services - Allied Health Services

⁠⁠⁠AccessCare Southern (ACS) prides itself on being a specialist provider of community case management, brokered services and individual planning and support for people who are older and frail, have a disability/disabilities or who have other complex care needs.

ACS also provides support to their Carers. ACS delivers services in the Local Government Areas of Kingston, Bayside, Glen Eira, Stonnington and Port Phillip. ACS receives funding from Victorian and Commonwealth Governments, and is auspiced by the Kingston City Council.

This tender aims to appoint a panel of Allied Health Services that share the same service objectives as AccessCare Southern. These include to:

* Provide coordinated therapy and care to meet the assessed needs of Consumers
* Support the needs of Consumers to be more independent at home and in the community thereby enhancing their quality of life
* Provide responsive and high quality services
* Provide value for money

AccessCare Southern is appointing a panel of Allied Health Services to ensure we can be responsive to the diverse and varied goals and needs of our Consumers and their Carers.
