Expression of Interest for the Diabetes Care Clinic are now open to all small and solo practices in the CESPHN region

⁠⁠⁠Overview

Central and Eastern Sydney PHN (CESPHN) is inviting Expressions of Interest from small and solo general practices across the CESPHN region to participate in the Diabetes Care Clinic program, delivered by Diabetes Australia.

Scope

The scope includes:

• Participation in the Diabetes Care Clinic program delivered by Diabetes Australia
• Access for the practice to diabetes-related allied health support delivered by Diabetes Australia, either within the practice or virtually, at no cost for eligible patients

Funding

• Financial support to the value of $5,000 (excluding GST) will be offered to participating general practices
• The program will run until June 2027

Eligibility

• Open to small and solo general practices across the CESPHN region
