WNSW PHN is seeking proposals for the delivery of a Wellbeing Guide service designed to improve access to non-clinical supports and community-based activities for older people who are at risk of or experiencing social isolation, and who face additional barriers related to geography, service navigation, inequity and limited local capacity.
The procurement is grounded in a social prescribing model; however, the commissioned service is not intended to operate as a simple referral or information service. The service is intended to function as a structured, person-centred navigation and engagement response for older adults with complex social, functional and wellbeing needs, who require more than passive signposting, and who do not consistently access, engage with, or sustain participation in existing community supports without additional assistance.
