Country to Coast Queensland (CCQ) is seeking a suitably qualified provider to deliver Balance for Life: Healthy Ageing and Falls Prevention, a community-based program for older people at risk of falls in the Wide Bay / Hervey Bay region. The program aims to improve strength, balance, confidence and social connection, while reducing falls risk and supporting healthy ageing.
The successful provider will be responsible for delivering a structured, community-based falls prevention program, including:
- Establishing and maintaining referral pathways (including self-referral) with primary care, hospital and community services
- Recruiting and assessing eligible participants
- Delivering a minimum of three defined program cycles per year in accessible community venues
- Engaging appropriately qualified facilitators and presenters
- Collecting and reporting performance and outcomes data
