Review of Investigation Management System

⁠⁠⁠Suppliers are invited by the Customer to submit an offer for Invitation to Offer No.RSHQ.2026.272 for the provision of an independent review of Resources Safety and Health Queensland’s (RSHQ) investigations management system.

This will include assessment of system capability, data quality, governance and reporting and the development of recommendations and an implementation roadmap to ensure the system is fit-for-purpose and supports regulatory decisions-making.

The Serious Incident Investigation Unit (SIIU) is the centralised, specialist unit for all RSHQ investigations of serious safety and health matters.

It is responsible for undertaking investigations into serious and complex matters that may constitute offences against and contraventions of the four Acts applying to Queensland’s resources industries:

(a) Coal Mining Safety and Health Act 1999

(b) Mining and Quarrying Safety and Health Act 1999

(c) Petroleum and Gas (Production and Safety) Act 2004

(d) Explosives Act 1999.

The types of matters the SIIU investigates include:

(a) a workplace death

(b) a serious injury

(c) occupation disease where an obligation holder has potentially contributed to the workers’ exposure to the cause of the occupational disease

(d) a serious incident involving a potential serious contravention of statutory obligations

(e) alleged conduct of a medical provider that is grossly negligent or deliberate in nature.

The SIIU was established to deliver specialist, high-quality investigations across RSHQ’s inspectorates, addressing prior issues of inconsistency, capability gaps, and inefficiencies. It is responsible for capturing, managing, and analysing investigation data and evidence to support compliance, enforcement, and continuous improvement across the resources sector. The SIIU prepares an investigation report and where required, a brief of evidence for submission to the separate Office of the Work Health and Safety Prosecutor.

Recent findings from an independent review of the regulatory model RSHQ is responsible for regulating, highlighted concerns with approaches to investigations and enforcement. RSHQ investigators have also documented challenges with current investigations management system arrangements, providing operational insight to be considered by the review.
