Tender Description

⁠⁠⁠This Request for Tender (RFT) seeks to commission a service provider to establish and operate the Mental Health Multidisciplinary Team Services Program (MHMDT) for Country SA PHN.

The MHMDT will serve to support the primary care system to safely manage and support people with complex mental health needs in the Riverland Mallee Coorong Local Health Network region.

Multidisciplinary team-based models must be evidence-based, holistic, and person-centric, with services tailored to meet identified regional and participant needs and be responsive as these change overtime. The MHMDT must operate as co-located entities within general practices or Aboriginal Community Controlled Health Organisations (ACCHOs) to provide timely access to care for persons in a familiar healthcare place, located in their community.

This activity will combine clinical and non-clinical mental health supports, for the delivery of better patient outcomes and avoidance of unnecessary hospital presentations for mental health crisis. Persons with 'complex mental health needs' (assessed as IAR DST 3 - 5) that are typically underserviced locally and routed to tertiary care will instead access the MHMDT within the primary care environment for up to 2 years per client. Services must work in collaboration with the Riverland Mallee Coorong Local Health Network and promote shared care with general practice, providing care coordination and 'wrap around' multidisciplinary support for people in a community setting.

The MHMDT must be inclusive, holistic and responsive to any person needing support, regardless of gender, age, sexuality, religion, ethnicity, disability, impairment, and culture.

The successful applicant is expected to actively collaborate with Country SA PHN, SA Health, including Local Health Networks (LHN), people with lived experience, and other key stakeholders in the ongoing co-design of the service model to be reflective of the community's need and context of rurality.

The Country SA PHN MHMDT Service Specification (Appendix D) should be used to inform the service model and ongoing co-design and establishment.

The establishment timeline will see the successful applicant work towards commencing a level of service delivery by 01 September 2026, or earlier where possible. Applicants will need to provide a proposal outlining how achievement toward key milestones and deliverables would be successfully achieved to meet the expected service commencement timeline.
