Brisbane South PHN is commissioning new placed-based Mental Health Multidisciplinary Services for People with Complex Mental Health Needs in Primary Care Settings through an open tender process, targeting Forest Lake, Inala, Oxley, Beenleigh, Loganlea and Eagleby.
The commissioned services will provide place-based, integrated clinical and psychosocial supports tailored to individual needs. A core requirement of the model is that multidisciplinary teams are co-located within participating general practices, enabling close collaboration with General Practitioner's (GPs) and practice teams and strengthening the role of primary care in supporting people with severe and complex mental health challenges. Services will include care coordination, service navigation, allied health interventions and psychosocial supports delivered by a multidisciplinary workforce. Through coordinated care teams embedded in general practice and working in partnership with Hospital and Health Services and local community organisations, the program aims to improve access to care, strengthen continuity and coordination of services, and support individuals to manage their mental health and wellbeing in the community.
