Tender Description
Country SA PHN invites submissions for the Youth Universal Aftercare Co-design and Consultation; to co-design an innovative aftercare model for people 12-17 years old in regional, rural and remote South Australia who experience suicidal crisis and suicidal behaviours.
The aim of the project to is to develop an evidence-based youth aftercare model for young people, increasing their access to support closer to where they live and potentially reduce the suicide rate within our communities. Country SA PHN had the 4th highest suicide prevalence for youth ages 12-17 years of the 31 PHN's in Australia and Country SA PHN seeks to reduce this rate through the delivery of evidence-informed support programs tailored to the needs of young people.
The co-design will include exploratory work and formal evaluation to aid in the ongoing development of a youth aftercare model which will meet the needs of suicidal youth and young people, within the context of regional, rural and remote communities.
Country SA PHN is seeking submissions from suitably skilled and experienced organisations who can design and facilitate a multi-faceted consultation and co-design project, which seeks to understand how Country SA PHN can reduce the suicide rates of youth and young people in country South Australia using the Universal Aftercare model.
Country SA PHN is seeking proposals for projects which are to be delivered between May 2026 to June 2027.
It will be important for the Model of Care to reflect evaluation findings, emerging evidence, or changing community needs, while maintaining fidelity to core components of Universal aftercare.
