CESPHN invites suitably qualified and experienced providers to deliver an evidence-based multiple health behaviour change (MBHC) intervention targeting modifiable risk factors for chronic disease in young people aged 13-25 years old* that incorporates E-Health principles.
Proposals will incorporate evidence-based strategies for behaviour change such as health coaching, motivational interviewing and peer groups or buddy systems. These strategies will be complemented by an E-Health platform with digital tools, which may include personalised digital plans, habit trackers and micro-lessons or modules, with systems to guide engagement and progress, such as push notifications and structured support pathways.
About the RFP
Central and Eastern Sydney PHN (CESPHN) are requesting proposals for an evidence-based multiple health behaviour change (MBHC) intervention targeting modifiable risk factors for chronic disease in young people aged 13-25 years old* that incorporates E-Health principles.
The intended objectives of this program are to provide young people and their parents/carers residing in the CESPHN region with an evidence-based program promoting healthy behaviour changes, with specific focus on
- Healthy eating
- Active living
- Reduction of screen time
- Reduction of harm from smoking and vaping
- Increasing access to lifestyle interventions for young people to reduce incidence of modifiable risk factors of chronic disease
- Providing an innovative approach to health promotion that adapts to the priorities and expectations of young people
CESPHN invite applicants to propose an innovative, person-centred approach that allows content and learning to be tailored to the individual needs of participants. This may include bundling content into streams that align with the persons particular goals (e.g. reducing metabolic or cardiovascular risk factors, targeting energy and mood, reducing use of tobacco, e-cigarettes and/or alcohol), prioritising participant choice and a flexible opt in/opt out approach that enables the person to choose the content based on their goals and interests.

The application should capture in Part C all the relevant financial components that are in scope for funding.
