In-person Primary Care After Hours Service for Older People

⁠⁠⁠COORDINARE - South Eastern NSW PHN is inviting proposals for an After Hours Mobile Primary Care Service to support older people across Eurobodalla and Bega Valley communities.

This RFP seeks providers who can deliver a flexible, mobile clinical service that responds to urgent but non life threatening health needs in the after hours period. The service will support:

• Older people living at home within community and in retirement villages \
• Residents in residential aged care homes

We welcome applications from organisations with innovative workforce models, including but not limited to:

• Nurse Practitioners
• General Practitioners and Medical Practitioners
• Paramedic clinicians

The model aims to reduce avoidable emergency department presentations, strengthen continuity of care, and enhance after hours options in regional communities where primary care access can be limited.

Project funding:

A total of up to $600,000.00 (ex GST) will be available and dispersed through this initiative. Projects supported under this initiative will run for a duration of approximately 12 months, commencing from May 2026 and concluding in May 2027.
