RFT - Connected Care - Mental Health Service North, North West and South

⁠⁠⁠Primary Health Tasmania seeks to commission suitably capable and experienced service providers to deliver the Connected Care - Mental Health Service across the North, North West and South regions of Tasmania.

This new service will form part of the Mental Health Continuum of Care (MHCoC) model and provide coordinated, evidence based mental health support aligned to IAR-DST Levels 2-4.
