Interim Care Capacity

⁠⁠⁠Gold Coast Hospital and Health Service (GCHHS) require a supplier that can provide 24-hour care services to support GCHHS patients that are awaiting finalisation of their discharge following a hospital episode of care, or that require ongoing care/therapy to optimise their function and independence before returning to their primary residence.

While these patients are medically fit for discharge from acute care, they often require extended inpatient stays due to their ongoing care needs. This challenge is compounded by the limited availability and accessibility of specialised community-based services to support them post-discharge. To address this gap, we are seeking providers who can offer additional inpatient bed capacity for sub-acute patients, ensuring safe, appropriate care until suitable transition arrangements can be made. The service must provide accessibility to guaranteed beds for placement of patients referred to the supplier by GCHHS.
