Bridges to Wellness

The PHN is looking to partner with suitable providers to co-design and deliver Bridges to Wellness, a new multi-disciplinary team service for people with severe and complex mental health needs. We are seeking to partner with providers in the Lower Hunter and Mid Coast regions.

About Bridges to Wellness

We are seeking to design and implement a multi-disciplinary service for people with severe and complex mental health needs. The service aims to provide holistic, person-centred, and long-term care through a multi-disciplinary team of health professionals including a general practitioner (GP).

Bridges to Wellness Key Features

• Multi-disciplinary team including a GP.
• Focus on holistic person-centred care utilising a bio-psycho-social model.
• Long term service with no minimum or maximum appointments accessible up to two (2) years.
• Complement existing local mental health services.

Co-design

To ensure that the service truly meets the needs of local communities, the PHN will partner with successful providers and local stakeholders to co-design the final service model. This will ensure the service meets the program objectives, budget and complements existing services.

The RFP for Bridges to Wellness is scheduled to be released in February via the etender portal:

• RFP-185 Bridges to Wellness (Mid-Coast)
• RFP-186 Bridges to Wellness (Lower Hunter)

We encourage providers to register with the etender portal to receive the RFP invitation and relevant notifications. Your organisation must be registered as a Supplier on the etender portal to receive the invitation and submit a proposal.
