Care Finder Program 2026-2029 Request for Tender

Country SA PHN has commissioned the delivery of the care finder program in country SA since 2023. Currently, the care finder program is delivered by several service providers with service coverage across all Country SA PHN regions.

Guided by best practice commissioning principles, including transparency, equity, value for money, and continuous improvement, Country SA PHN is conducting a formal competitive tender process for the ongoing delivery of the care finder program across all Country SA PHN regions from 2026-2029. Undertaking a tender process at this time provides an opportunity to strengthen service delivery by encouraging innovation, enhancing value, and supporting sustainable models of care.

Country SA PHN invites applications from suitably qualified and experienced organisations, who have the capacity and capability to deliver the services. When providing information regarding capacity and capability, Applicants should assume that the persons evaluating the Submissions have no prior knowledge of the Applicants activities, experience or any previous work undertaken.

If a change in service provider occurs as an outcome of the tender process, this will necessitate a transition of service and transfer of any active/open care finder clients between the current provider and the incoming provider. If required, transition of the care finder service will commence from May until 30 June 2026, with the program to be fully operational and services transitioned across from the current provider not later than 1 July 2026.

Applicants will need to demonstrate how they would deliver the care finder service and potentially support effective transition of service delivery within a short period of time with minimal risk to those currently accessing the service.
