Tender Description
The SMBI Child Health Connector (CHC) program is one element of this model, which seeks to improve connections with health for families and remove barriers to them accessing child health and specialist services for their children in a coordinated, safe way. Over the past four years, the model has been designed, operationalised, and refined locally, with indicators suggesting the model is achieving its intended outcomes for the community and partners. Brisbane South PHN is seeking a new provider to deliver this service on SMBI.
