For projects in the Central Australian and/or Barkly regions that address some or all of the goals of the Greater Choices for at Home Palliative Care (GCfaHPC) Program.
What is the tender?
A grant from NT PHN, valued at $567,000, will be available for works completed between Feb/March 2026 and June 2029. The funding is for projects in the Central Australian and/or Barkly regions that address some or all of the goals of the Greater Choices for at Home Palliative Care (GCfaHPC) Program.
The Greater Choices for at Home Palliative Care program aims to:
- improve access to the best palliative care at home
- support palliative care services in primary health and community care
- make sure Territorians get the right care, at the right time and in the right place to reduce unnecessary hospital visits
- generate and use data to improve services
- use technology to provide flexible and responsive care, including after-hours care.
These aims are supported by activities such as:
- Improving quality and coordination of primary health care
- Supporting measures that improve the coordination and integration of health services to manage health in the community with a focus on complex and chronic conditions, reducing potentially preventable hospital attendances and admissions
- Supporting the delivery of health information, advice and services through interactive communication technology to help people care for themselves and their families
- Support the workforce development, quality improvement and data development activities in the area of palliative care
- Promote the use of tools and expertise available through the National Palliative Care Projects
- Support uptake and use of advanced personal plans (APPs).
