About the RFP
Central and Eastern Sydney PHN (CESPHN) is requesting proposals for an allied health service to provide multidisciplinary support to general practices (particularly small and solo practitioner general practices) across areas that include Canterbury SA3, Strathfield-Burwood-Ashfield SA3 and Marrickville-Sydenham-Petersham SA3 for people living with prediabetes and type 2 diabetes. The allied health services will be focussed on diabetes education and support. All members of the allied health team will have relevant knowledge and experience in caring for people with prediabetes and type 2 diabetes. The funding amount is intended to cover salary and wages of allied health professionals and program delivery costs. Diabetes-related allied services provided under the program will be free of charge for patients and participating general practices.
The objectives of the multidisciplinary care model are to:
a. Improve access to allied health services for people living with prediabetes and type 2 diabetes in areas that include Canterbury SA3, Strathfield-Burwood-Ashfield SA3 and Marrickville-Sydenham-Petersham SA3
b. Reduce or delay the progression of prediabetes to type 2 diabetes through early intervention and person-centred, multidisciplinary care and education
c. Reduce the risk of complications, hospitalisation and mortality for people living with type 2 diabetes through person-centred diabetes management
d. Improve integration of allied health services and professionals in primary care, particularly small and solo practitioner general practices
The commissioned allied health service will deliver a service that incorporates:
e. A schedule of individual consultations with allied health professionals for eligible patients based on the clients needs, including goal setting, education and progress monitoring
f. Fulfilment of the allied health elements of the annual cycle of diabetes care, as a minimum, for patients with type 2 diabetes. Additional services may be appropriate depending on the clients needs
g. Lifestyle intervention strategies and education to address existing risk factors for prediabetes and other chronic diseases
h. Optional: an evidence-based group education program to complement, not replace, individualised education and existing programs such as the Get Healthy Service and Diabetes Australia group education programs
