Tender Description

Central and Eastern Sydney Primary Health Network (CESPHN) is requesting proposals from community centres or neighbourhood centres in the Eastern Suburbs region of CESPHN for the establishment of a community-based healthy ageing hub. The hub will promote positive ageing and support primary health care through the provision of social care and assistance to help older people with navigating health, aged care, social and welfare services.

Funding of $335,000 is available over a two-year period to support the establishment and operation of the hub.

The model of care is designed to build community capacity around healthy ageing and assisting general practice with provision of social care. The successful community centre or neighbourhood centre will employ a social or welfare worker dedicated to promoting healthy ageing and supporting older people to live well in the community.

Community and neighbourhood centres are ideally placed to host Healthy Ageing Hubs due to their strong focus on community development, established local networks, and trusted relationships with older residents. These centres play a vital role in promoting health and wellbeing and are well-positioned to integrate the hub into their existing service offerings.

To date, CESPHN has successfully established Healthy Ageing Hubs in Gymea, Newtown, and Ashfield, serving the Sutherland Shire and Inner West LGAs. This tender represents an opportunity to expand the initiative into the Eastern Suburbs region.

Central and Eastern Sydney PHN (CESPHN), invites eligible applicants to apply for the Community based Health Aging Hub initiative in the Eastern suburbs of Sydney.
