RACH Nurse Practitioner- Dementia Care

Summary Primary care Nurse Practitioners specialising in dementia care are increasingly recognised as essential connectors between general practice, aged care, and hospital outreach services. CESPHN invites applications from Residential Aged Care Home (RACH) providers operating five or more facilities within the CESPHN region to deliver a commissioned, Nurse Practitioner-led dementia behaviour support program.

This 18-month initiative aims to enhance dementia care across multiple RACH sites by embedding advanced clinical expertise, improving behavioural management, and building workforce capability. Providers with the capacity to coordinate service delivery across their network and a commitment to person-centred dementia care are encouraged to apply.

The program aims to improve dementia care outcomes, reduce hospital transfers, and build workforce capability through clinical support, education, and systems improvement.
