Improving Primary Care Access for Aged Care Residents in Thin Markets

Healthy North Coast's Residential Aged Care - Enhanced Primary Care Access Grants, are designed to support innovative and local solutions to address lack of primary care services for permanent residents in residential aged care homes in "thin markets" ie locations where lack of aged care primary care services is an impediment

These grants, up to $90,000 per project are available to Residential Aged Care Homes (RACHs) and/or primary care providers offering services in regions where GP shortages significantly impact the health and wellbeing of older people. The program supports innovative, locally co-designed solutions that either build capacity in the delivery of primary care services to residents or strengthen the capacity of aged care homes to meet their residents' evolving care needs.

Expressions of Interest are now open and will close at 9am on 31 October 2025. Eligible applicants are encouraged to propose place-based models of care, including virtual, hybrid, or multidisciplinary approaches, or capacity building initiatives that respond to the unique challenges of their communities. Projects must demonstrate collaboration, co-design, cultural safety, and sustainability beyond the grant period.
