Adelaide PHN

Adelaide PHN is one of 31 independent organisations nation-wide that are funded by the Australian Government to coordinate and improve primary health care services in their local areas.

The Australian Government's objectives for Primary Health Networks (PHNs) are that they will:

  •  increase the efficiency and effectiveness of health services for patients, particularly those at risk of poor health outcomes; and
  •  improve coordination of care to ensure patients receive the right care in the right place at the right time.

Adelaide PHN's catchment area covers the Adelaide metropolitan area, reaching from Sellicks Hill to Angle Vale and from the foothills to the sea. It encompasses a community of some 1.2 million people.

Adelaide PHN aims to improve health outcomes for the community by commissioning services to address local needs. It is guided by community, clinical and stakeholder input.

Background of the project

Adelaide PHN is seeking to commission a General Practitioner to deliver clinical services at Swallowcliffe Primary School, as part of the broader Children in the North Alliance. This initiative aims to improve health, development and wellbeing outcomes for children and families in disadvantaged communities through a place-based, community-led approach.

The Swallowcliffe Primary Health Care Program complements existing services at the Swallowcliffe Children's Precinct, including paediatric nurses, allied health professionals, and community engagement officers.

Description of Requirement

Through the GP Services for Swallowcliffe Primary Health Care Program Adelaide PHN is seeking to commission a suitably qualified General Practitioner (GP) to deliver;

  •  Weekly or fortnightly in-reach clinical services (4 hours/week or 8 hours/fortnight)
  •  Family-centred care addressing complex paediatric and family health needs
  •  Integration with existing school-based health services
  •  Participation in stakeholder working groups and education sessions

A joint application between two GPs at the same clinic will be accepted and is encouraged.

There are two key elements of the project.

Element 1, Clinical Services / appointments:

  •  GP offers 30-minute appointments to establish rapport and referral to suitable multidisciplinary providers i.e. OT, MH clinician etc. and address the intergenerational health literacy, experience and wellbeing of the community
  •  GP to see small number of families each week to address complex patient characteristics for the entire family (8 appointments across 4 hours)

Element 2, Non-Clinical - Relationship formation, education and sustainability:

  •  Relationship formation with school, community and health care providers as well as other local general practices (to share the load and create a sustainable system)
  •  Support and educate the wrap around staff to understand the principles to working with complex children. Due to high rate of DNAs need to be able to deliver clinical education support, engagement with teachers, school staff across various topics.

Key Responsibilities:

  •  Deliver 30-minute clinical appointments
  •  Provide referrals to multidisciplinary providers
  •  Support wraparound staff with clinical education
  •  Build relationships with local general practices for continuity of care

Outcomes:

  •  Increased access and uptake of primary care for children
  •  Improved health literacy and wellbeing of families and children with complex conditions and needs
  •  Strengthened partnerships between education, health, and community sectors

The services are to be delivered for the period until 30 June 2026.

Project Budget

Element 1- Medicare billings for all clinical appointments

Element 2 - Total funds of $35,000 have been approved for the provision of non-clinical services under the GP services of the project for FY 25-26. This includes:

  •  Covering any missed revenue from DNAs
  •  Create and provide resources for the Swallowcliffe community
  •  Support and educate the wrap around staff to understand the principles to working with complex children
  •  All equipment and transport costs
  •  Establishing relationships with local providers and transitioning of care
  •  Participate in project governance meetings
  •  Reporting and case studies to Adelaide PHN
