Brokered Services for Transitional Aged Care in the Coffs Clinical Network

⁠⁠⁠Coffs Clinical Network has 32 TACP packages, covering the areas of Macksville, Dorrigo, Bellingen, and Coffs Harbour. The services that are to be provided at these locations are non-clinician services

Project goals

The objective of this Tender is to seek qualified organisations to provide Non-Clinical services to clients across the Coffs Clinical Network associated with the Transitional Aged Care Program (TACP) Services.

Scope

Brokered support may include:

• Personal Care - Self-care tasks including bathing, dressing, grooming and monitoring of medication
• Domestic Assistance, including but not limited to - House cleaning & Washing and ironing
• Transport - to and from medical appointments, to shops, and to facilitate social interaction
• Respite Care - Short-term in-house respite for carers
• Social Support- Accompanying clients to appointments, telephone monitoring, assistance with shopping, General household support, such as paying bills
• Meals - Assistance with food preparation. The Brokered Service Provider will undertake the end-to-end process of identifying, sourcing, and engaging suitable candidates to provide the brokered services in a manner that ensures the continuous delivery of services.
