We are seeking suitability qualified and experienced Supplier(s) to deliver brokered services for our community-based patient care used by our Austin Health Community Programs.

Austin Health invites Suppliers to provide equitable access to community-based services for individuals, utilising the Austin Health Post Acute Care, Hospital in the Home and Transition Care (home based) services. The aim of the PQQ is to ensure governance over the safety and quality of care provided to consumers of this program, identify and partner with Tier 2 Suppliers that can provide services.

The aim of the questionnaire is to:

• Identify suppliers with experience in delivering brokered services (particularly nursing) to a large metro tertiary healthcare service

• Identify the number of suitable suppliers in the Australian market

• Identify suppliers who wish to enter into a longer term contract with Austin Health

• Understand service rates to inform budget builds

Following the evaluation of the PQQ submissions, suitable suppliers may be asked to enter into a non-exclusive service agreement as an approved Supplier. Further information on this next stage will be shared with these shortlisted suppliers.
