Northern Adelaide Local Health Network (NALHN) requires the procurement of a Digital Community Healthcare Management Platform to deliver a fit for purpose program that will support an integrated approach to realization of the benefits of In-Home Care and Hospital Avoidance Programs for the whole of NALHN Community based services including Safe at home, Aged Care, Rehabilitation and Palliative Care (ACRPC), and Women’s and Childrens Division (WCD).
