⁠⁠⁠Good afternoon,

Can we please request a quote? kindly refer to the details below and the attached document/s. Please ensure your quote covers the tech specs as per the Housing requirements.

Please note that your quote must:

1. Review the Scope of Works and inform us of any inconsistencies, errors, incomplete information, and/or clarifications ASAP. This must be done BEFORE you submit your quote.

2. Include a price breakdown per trade. Please use the MDT RFQ price breakdown template

3. Include your estimated SDP, and if applicable

4. Indicate if you require progress payments, including milestone details

5. If not able to quote, send a reply explaining briefly your reason/s:

Submission of quotes should be made no later than 5:00 PM Monday, 04/08/2025

Please find attached the documentation, which must be referenced when preparing the quote.

Progress payment: Indicate (ü) whether a progress payment arrangement is to be considered:

• Yes
•  No

*Progress payment terms MUST be recommended by the Head Contractor at the time of submitting quotes to the Principal. These terms are negotiable, however, they must be agreed upon with the Principal prior to any works being awarded.

In addition to the above request, the Housing Authority also requires the following:

• A required price breakdown as per the attached
• All ACM Pricing needs to be identified separately per property (if applicable)
• Forward the quote -- together with all supporting documentation to the Team mailbox (pandf@lakemaint.com.au)
• A draft Service Delivery Program (SDP) needs to be supplied together with the quote
• Please provide a lead time (in weeks) to mobilise to the site, and, a lead time (in weeks) to complete all works as per SOW
• All subcontractors, their employees, and contractors MUST complete onboarding inductions prior to commencing work on site

 Important:

The requirements set by DoC must be followed and reflected in your quote (refer to the details below and attached documents). A quote deemed incomplete will be rejected. Likewise, works will only be deemed completed and paid after all scope requirement/s are submitted (i.e. before and after photos, reports, etc.)

Kindly remember: Subcontractors are to follow the appropriate protocols when entering/leaving remote communities.
