Tender Description

Country SA PHN is seeking responses to this Request for Proposal (RFP) from suitably qualified and experienced organisations. The successful applicant/s will design, implement and evaluate new, innovative projects which aim to support healthy ageing amongst older people who live at home in the community.

Ageing Well in the Community activities will support priority populations of older people to access early intervention services or programs designed to improve health and wellbeing, maintain independence and reduce risk of frailty and deterioration earlier in the ageing trajectory compared with traditional aged care programs.

This activity aims to target older people aged 50 years or over who live independently in the community. Older people living in Residential Aged Care Homes (RACHs) are out of scope of this activity.

Activities must include a focus on key risk factors and determinants of health for older people including but not limited to frailty, social isolation, health literacy and digital connectivity.

Activities must include strategies and activities which focus on engaging with priority populations of older people including:

  • Aboriginal and Torres Strait Islander Peoples
  •  Lesbian, Gay, Bisexual, Transgender, Intersex and Queer (LGBTQIA+) older people
  •  Culturally and Linguistically Diverse (CALD) older people
  •  Older people with disability
  •  Older people experiencing financial disadvantage
  •  People providing informal care to older people

Activities must be delivered between September/October 2025 to 30 June 2027. Applicants are able to apply for projects delivered across single or multiple financial years i.e. 2025-26, 2026-27 or 2025-27.

There is a preference for projects that support intersectoral partnerships to facilitate integration and connectivity across systems.

Projects must not duplicate current Country SA PHN commissioned activity, or services that would be more appropriately funded by other funding sources (i.e. My Aged Care).

Activities must be delivered in one or more of the following local health networks (LHNs):

  •  Barossa Hills Fleurieu LHN
  •  Eyre and Far North LHN
  •  Flinders and Upper North LHN
  •  Limestone Coast LHN
  •  Riverland, Mallee, Coorong LHN
  •  Yorke and Northern LHN

Applicants who intend to apply for more than one region are required to complete separate applications for their chosen regions. It is expected that each separate submission would be tailored to the individual regional context.
