Tender Description
HabitHack is a new web app aimed at supporting young people in mental health care to prevent re-hospitalisation after their discharge. Its primary goal is to reduce 29-day readmission rates for EMyU patients and enhance their ability to maintain positive behaviours.
While the app is mainly designed for mobile phones and tablets, a portion of users will also access it via desktop computers.
