Grants of up to $5,000 are available to general practices based in Bankstown, Fairfield and Liverpool LGAs to improve COVID, flu and childhood vaccination rates to vulnerable communities.
Through this expression of interest, successful practices will support conversations with patients about COVID-19, influenza and general childhood vaccinations to improve health literacy among vaccine-hesitant patients.

Application and funding timeline
Activities can begin once the Letter of Agreement has been signed and must be completed by 11 August 2025.
To receive payment, final invoices must be submitted to SWSPHN by 30 October 2025.
Activities covered by the grant
Practices are expected to undertake the following activities:
- hold conversations with vaccine-hesitant patients to improve understanding of COVID-19, flu and childhood vaccines, and assist them to access vaccination if appropriate
- set aside time to use clinical software to identify under-vaccinated patients from vulnerable groups, recall them and discuss the benefits of vaccination
- connect eligible patients with a COVID-19 vaccine provider
- provide vaccinations to eligible patients from vulnerable groups
Activities may be delivered face-to-face or via telehealth, and can include both individual and group-based sessions.
Eligible vulnerable groups
People who may be missing some or all of their COVID, flu or childhood vaccines include:
- First Nations people
- older people
- people with chronic conditions
- people from culturally and linguistically diverse (CALD) communities
- people who are homeless or at risk of homelessness
- people with a disability
- children aged five and under
Reporting requirements – minimum data set
Practices must complete a supplied reporting spreadsheet template with the following information:
- total number of conversations with people from vulnerable groups
- number of patients in each vulnerable group
- at least 10 conversations with people from CALD backgrounds
- at least one good news story
- number of vaccines given, listed by type (COVID, flu, childhood)
Invoice and payment details
- practices will be paid $100 per patient
- invoice after reaching 25 patients, then again after 50 patients
- payments will be made in $2,500 increments (ex GST)
- submit the reporting spreadsheet along with each invoice
- invoices and reporting submitted after 31 October 2025 will not be accepted
- payment will be made a month after invoice and minimum data set received
