Country SA PHN is seeking responses to this Request for Tender (RFT) from suitably qualified and experienced organisations. The successful applicant will be responsible for developing, managing, and delivering the Local Link Service, facilitated by a Local Link Team (LLT). This service will provide integrated support to primary care providers and patients in the Flinders and Upper North Local Health Network (LHN) pilot site regions in response to Family, Domestic, and Sexual Violence (FDSV).

A dedicated LLT will deliver three core functions:

1. Primary care support, offering trauma-responsive guidance and case consultation;

2. Patient support, ensuring coordinated care, system navigation, and safety planning; and

3. System integration, fostering collaboration between services and addressing system gaps.
