Primary Health Networks (PHNs) have been established with the key objective of increasing the efficiency and effectiveness of health services for patients, particularly those at risk of poor health outcomes, and improving coordination of care to ensure clients receive the right care in the right place at the right time.

The Country to Coast Queensland (CCQ) region spans 161,108km2, covering Darumbal, Gangulu, Bayali, Wadjigu, Bayali, Gayiri, Goreng Goreng, Gureng, Taribelang Bunda, Butchulla, Waka Waka, Wuli Wuli and Gubbi Gubbi Nations. This begins as far south as Glasshouse Mountains and stretches north of Yeppoon; west beyond Emerald, encompassing Rockhampton, Hervey Bay, Maryborough, the Sunshine Coast and its Hinterland, Gayndah, Gladstone and Bundaberg.

PHNs deliver three core functions:

1. Coordination and integration of local health care services to improve quality of care, people's experience and efficient use of resources

2. Commissioning services to address population health needs and gaps in service delivery and to improve access and equity

3. Capacity building and practice support to primary care and mental health providers to support quality care delivery.

CCQ is committed to supporting the readiness of the aged care sector for digital transformation and meaningful use of digital systems and technologies. By this request for proposal CCQ seeks proposals from organisations with the understanding of and ability to assist RACHs throughout the MHR registration process, as well as the expertise to conduct pre-audit assessments of the specific related strengthened Quality Standards outcomes, for 14 identified RACHs across our region of the Sunshine Coast, Wide Bay, and Central Queensland. Of the 14 homes 1 is located in Rockhampton, 1 in Gladstone, 1 in Childers, 1 in Hervey Bay, 2 in Noosa, and 8 on the Sunshine Coast.
