There are 31 Primary Health Networks (PHN) throughout Australia which are independent not-for profit commissioning organisations funded by the Commonwealth Department of Health to improve the health outcomes of the region. Country to Coast Queensland (CCQ) delivers the PHN program within the Central Queensland, Wide Bay, Sunshine Coast region, guided by its vision to 'support our region to live their healthiest lives'.
CCQ is committed to enhancing disaster preparedness and building community resilience through strategic planning, targeted support programs, and strong partnerships, in order to protect and promote health. Our efforts have focused on both immediate response measures and long-term resilience strategies. The CCQ Disaster Resilience Plan 2024-2025 outlines a structured and coordinated approach with our partners and community to enhance disaster resilience across the region.
The primary objective of the Resilient Care Program is to ensure people are healthy and well, wherever they live and regardless of how the climate changes and the disasters which occur. The key result areas for the program are to:
1. Strengthen the resilience of local healthcare providers and systems.
2. Build local resilience to the health impacts of climate and disasters.
3. Enhance the integration and role of primary healthcare providers and support active community involvement in all phases of disaster management.
This procurement jumpstarts the program by identifying and co-designing quick-win initiatives and providing funding and support to collaborations to bring them to life.
The aim of these Resilient Care Grants is to commission grants that fund activities that deliver or assist to build initiatives that support everyone to be healthy and well despite disasters and climate hazards, with a particular focus on older people (particularly those who live independently at home in the community), people with a disability (particularly intellectual disability), and enhancing the primary healthcare systems capability to safeguard community health in response to climate and disaster hazards within the target Local Government Areas (LGAs) of Livingstone, Rockhampton, Gladstone, Bundaberg, and Fraser Coast.
