DDWMPHN is seeking to commission an Integrated Mental Health Hub in Toowoomba which will provide a front-door to the mental health, suicide prevention, alcohol and other drugs (MHSPAOD) system in Toowoomba with a 'no wrong door' approach.

The hub will deliver services across the stepped care continuum inclusive of low, mild-moderate, and severe and complex mental health supports enabling step-up/step down care as required during recovery.

The model will include wrap around psychosocial support, particularly in relation to people with severe and complex mental health needs that require both clinical and psychosocial care.

Warm referrals and connections to the wider MHSPAOD system within the region will be facilitated, including and beyond PHN commissioned services.

The successful provider/s will work with the PHN, regional partners and broader system stakeholders to establish a place-based healthcare neighbourhood and ecosystem of support services within the Toowoomba LGA.
