Tender Description
Evaluations of the after-hours homelessness health programs commissioned by Brisbane South PHN since 2018 found that a nurse-led outreach healthcare model delivered high-quality care for people experiencing homelessness while being more cost-effective than a GP-led outreach model. As a result, a new nurse-led outreach healthcare model will be commissioned to deliver primary healthcare to those experiencing homelessness and that are vulnerably housed in the Logan Local Government Area.
