Tender Description
Based on the HNECC PHN Palliative Care Needs Assessment findings, the PHN invites suitably qualified and experienced organisations with the capacity to deliver a pilot end-of-life care coordination service.
The pilot program aims to offer care coordination and assistance in navigating services for individuals with a palliative diagnosis and minimal social support during their end-of-life phase, with the objective of receiving care and dying in the comfort of their own homes.
