Tender Description
The PHN invites suitably qualified and experienced organisations with the capability and capacity to deliver the Head to Health Muswellbrook Service.
The Head to Health adult mental health services are to be incorporated into a central intake and assessment phone service for triage and transfer of referrals to appropriate services within a stepped care model that provides multi-disciplinary, holistic, and collaborative care.
The service is to be based in Muswellbrook that provides the Head to Health services within the surrounding LGAs of Scone and Singleton.
