Tender Description
The HNECC PHN invites suitable, qualified Providers with capacity and capability to submit proposals to deliver early intervention initiatives to older Australians within the identified geographical area.
Some older Australians enter aged care earlier than they may otherwise need to, due to a lack of support for healthy ageing or inability to manage their chronic conditions in the community.
The objectives of the funding are to:
- support senior Australians to live in the community for longer (including those not currently receiving aged care services) through commissioning early intervention initiatives that promote healthy ageing, slow decline and support the ongoing management of chronic conditions.
- increase awareness in the primary health care workforce of the needs of the population, and
- Increasing availability of early intervention initiatives.
Providers must demonstrate that they have existing local community connections with, and specialist skills and experience in supporting older people.
Organisations seeking to expand existing capacity and/or existing reach through additional funding would be strongly encouraged to respond.
