Tender Description

⁠⁠⁠ The South Metropolitan Health Service (SMHS) requires the provision, hosting and ongoing support of an integrated Software as a Service (SaaS) application. The application will facilitate early screening by allowing patients to complete a Patient Health Questionnaire (PHQ). The application will screen for risk factors, apply algorithmic clinical decision support systems and track progress of defined markers.

The aim of the program is to optimise the health of a patient throughout their surgical journey by reducing the impact of chronic disease and improving the preparedness of all patients through individualised, consistent assessment, referrals and early intervention. The application will be available as both a desktop app and mobile app and will use visual indicators to enable clinicians to assess patient status in real time to support a clinical response.
