CESPHN is requesting proposals from community centres or neighbourhood centres in the Inner West, Canterbury, Georges River or Sutherland Shire regions. CESPHN will provide funding to community centres or neighbourhood centres to establish a healthy ageing hub through employing a social or welfare worker dedicated to promoting healthy ageing and supporting older people to live well in the community.
Healthy ageing hubs will provide a local and face-to-face drop-in service for community members to receive practical assistance and guidance on a range of issues to support positive ageing and social connection, and assistance in navigating programs and services available in their local area. Healthy ageing hubs will also offer a much-needed support to general practice, where there is often a lack of available time to liaise and negotiate with organisations and professionals to ensure all aspects of an older person's needs are met. This initiative will provide an important conduit between primary health care and social care systems for older people.
The objectives of the community-based healthy ageing hub are to:
i. Provide navigation support and guidance on healthy ageing to help older people resolve issues and connect with aged care services and healthy ageing programs in their local area.
ii. Support healthy ageing and age-friendly neighbourhoods through education that improves the knowledge of older people and supports them to maintain good health and stay socially connected.
iii. Support the local primary health care workforce by providing a mechanism for them to refer their patients who would benefit from the assistance of a social or welfare worker.
