Residential Aged Care Facilities Telehealth Support Grants

⁠⁠⁠Country SA PHN is providing supporting country Residential Aged Care Facilities (RACF) with the costs associated with establishing suitable telehealth solutions by offering Telehealth Support Grants. This will increase the access to telehealth care for use by primary health care providers such as GPs, Nurses and Allied Health professionals, who will be able to provide patient care virtually, benefitting the residents, RACF staff and the primary care providers.

The RACF Telehealth Support Grants are up to $8,000 (Excl. GST) per RACF. Grants may be used for IT

Hardware or Software to support telehealth consultations nd this could include:

• iPad
• Tablet
• Laptop (Wi-Fi and Internal Camera Supported)
• Web-Camera
• Microphone
• •Speakers
• LCD Screen / Computer Monitor / Extended Display screen for Laptop
• Headset
• Technology Accessories such as HDMI cord and more.
• Education and Support

RACFs need to utilise the grants funding to purchase the equipment and complete the configuration process. Once off configuration support can be requested within the application form. Thereafter configuration and management assistance will be the responsibility of the RACF.

Selection Criteria

To be eligible for the RACF Telehealth Support Grants the organisation must:

• Be within the Country SA PHN region.
• Complete the online application form to be assessed by the Ageing well team and the next steps will be instructed.
• Declare that the funds will be spent within accordance of the guidelines, and they will provide a
  performance report of what the funding was utilised for and its outcomes.
• Complete establishment of Telehealth Care within 6 months of accessing the Grant.
