CESPHN is seeking tender applications for the establishment of a network of care finders to provide specialist and intensive assistance to help people within the care finder target population to better understand and access aged care and connect with other relevant supports in the community.
The Royal Commission into Aged Care Quality and Safety found that the aged care system needs more localised and face-to-face support to help people to navigate and access aged care services and other relevant supports, particularly for those at risk of falling through the cracks.
The care finder program was announced in the 2021-22 Budget as part of the Connecting Senior Australians to Aged Care Services measure and will be delivered through PHNs. Care finders will:
i. provide specialist and intensive assistance to help people to understand and access aged care and connect with other relevant supports in the community and;
ii. specifically target people who have one or more reasons for requiring intensive support to interact with My Aged Care (either through the website, contact centre or face-to-face in Services Australia service centres), access aged care services and/or access other relevant supports in the community.
