Tender Description

⁠⁠⁠This request for proposal (RFP) has been developed by the Western New South Wales Primary Health Network (WNSW PHN) as part of the commissioning process to select and engage organisations to deliver the Integrated Team Care Program across Western NSW and Far West NSW. The geography for service provision has been divided into 6 Lot areas based on Local Government Areas aligned to Regional Assembly boundaries. The work will include assisting individual networks to plan and undertake initiatives in their local community as well as supporting the networks to form a collaborative approach. The project is being undertaken as part of Integrated Team Care (ITC) Care Coordination and Supplementary Services funded by the federal government.

This document provides contextual and technical information for organisations interested in submitting a proposal to undertake the project. It firstly provides background information about ITC and the redesign of the ITC program commissioned by WNSW PHN. It then describes the aims and objectives of the proposed project and the roles and responsibilities that the successful organisation will have. The RFP concludes with information about the indicative funding amount, process for submitting proposals, assessment criteria, timeline and general conditions.

The Integrated Team Care (ITC) program is intended to improve the health outcomes of Aboriginal and Torres Strait Islander people with chronic disease. This is a critical program in the Western NSW region, where 10.5% of the population identify as Aboriginal or Torres Strait Islander (please note, for the remainder of this document the term Aboriginal will be used reflecting the population across the western and far west NSW area). WNSW Primary Health Network (WNSW PHN) has the highest proportion of people who identify as Aboriginal in NSW. Overall, the Aboriginal population in the region experience poorer health outcomes compared to the population as a whole. Access to services and transport are also major challenges for communities in the Western NSW region, along with variable availability of access to culturally appropriate primary care services.

In this context, WNSW PHN has undertaken significant consultation to inform the redesign of the ITC program. The redesign aims to build on the previous success of the program in delivering positive outcomes for consumers, continuing to build the health and wellbeing of communities across the region and to support the ongoing development of the Aboriginal health sector.
