Tender Description
The PHN invites suitably qualified and experienced Providers to submit a proposal to deliver the Veterans Care Navigation Pilot Program on the Central Coast. Services will be established in a pilot capacity with and establishment period commencing November 2022.
The successful Provider will be offered a contract for services to be operational by January 2023. The contract end date is 31 December 2024.
The aim of the Veterans Care Navigation Pilot Program is to see a reduction in mental health disorders and suicide among veterans and their families through:
- The ability to navigate access to the right services at the right time (health, social and community);
- The ability to overcome barriers in accessing care; and
- Increased health literacy and self-management.
Services and activities are to be provided to veterans living on the Central Coast. It is expected that Services are provided by 2 x FTE non-clinical Care Navigators
This will be achieved through engagement with the Veteran Care Navigation Pilot Program which demonstrates ongoing relationships with local General Practices/Medical Centres, Allied Health Professionals, Community Services (Legacy, Open Arms, RSL, DVA). The Care Navigators will receive referrals from GP's, Allied Health Professionals, Community Services, peer support groups and veterans and their families.
