Supply of Craniomaxillofacial, Neurosurgery and Small Bone Implants (fixtures or prosthesis) and Associated Consumables to Western Australian Health Service Providers (HSPs)

⁠⁠⁠The Request will invite offers for the supply of Craniomaxillofacial, Neurosurgery and small bone implants (fixtures or prostheses) and associated consumables to Western Australian Health Service Providers.  The scope of the Contract will include a range of products and associated training, education and/or support.

Respondents shall nominate the categories of Products they wish to offer, and may offer one or more Products under each category nominated. Each category of Products will be considered independently and, in order to adequately account for clinical end user requirements, a single supplier or panel of suppliers may be appointed for individual line items within each product category and/or subcategory.  The Contract will operate as a standing offer arrangement (Head Agreement), with orders (Customer Contracts) being placed on an ‘as required’ basis during the Term of the Head Agreement.

The scope of the proposed contract will include the product categories/subcategories listed below and allow provisions for adult, paediatric and neonatal patients where applicable.
