State Library Victoria (SLV, the Library) seeks Offers to this Request for Offer (RFO) SLVT 111 to collaborate with a suitably qualified, innovative and professional Audio-Visual contract partner who will enhance the delivery of high quality events and programs. Through the provision of staff, services and specialised equipment the successful contractor will work in partnership with the Library and key stakeholders of the Library including sponsors, program partners and external clients.

The RFO documentation attached in the Specification Documents tab comprises the Request For Offer_Parts A-D_SLVT111 Audio-Visual Services document (the RFO document), and:

• RFO_Part D_Offer Document_SLVT111_Audio-Visual Services (the Offer Document),
• The following as referenced in RFO Part B – Specification in the RFO document:

1. Appendix C - SLV AV Venue Specification (1), and
2. Appendix D - SLV AV Equipment List (As built), and

• The following as referenced in RFO Part C – Proposed Contract in the RFO document:

1. Standard Library terms and conditions of contract Goods Services agreement (long-form),
2. The standard Library Deed of Confidentiality, and
3. The following standard Library contractor induction and on site compliance forms, permits and manuals applicable for all suppliers and their sub-tier suppliers and sub-contractors who would have personnel working on site if awarded the Contract:

• Contractors Application for Registration,
• Contractors Induction Form
• Contractor Induction Manual,
• Contractors OHS Questionnaire,
• Emergency Procedures and Evacuation Information, and
• General Work Entry Permit.

Please:

1. Review the attached RFO documentation and raise any enquiries regarding the RFO to the Online Forum at least 1 day before the Forum Closing Deadline (1:00PM Australian Eastern Standard Time (AEDT), Tuesday 24 August 2021),
2. Submit a compete RFO response in accordance with the RFO document and the terms and conditions referenced therein no later than the Tender Closing Date (1:00PM Australian Eastern Standard Time (AEDT) on Tuesday 31 August 2021), and
3. Reference the RFO number SLVT111 on all correspondence regarding this RFO.
