Community Nursing In-Home Falls Assessment

Central Queensland, Wide Bay, Sunshine Coast PHN (the PHN) seeks to contract an established, community-based Health Care Provider to employ one full time equivalent (FTE) Qualified Registered Nurse(s), to deliver in-home falls assessments to older clients, as part of the Fraser Coast Falls Prevention Service referral pathway.

Eligibility: This Tender is ONLY open to community-Based Health Care Providers who currently deliver in-home care services to older clients within the Fraser Coast area.

The PHN and the Wide Bay Hospital and Health Service have co-designed the Older Person's Falls Initiative program and as a result the Fraser Coast Falls Prevention Service referral pathway was established to reduce the personal and health system impacts associated with older person's falls. The initial complex referral section of the pilot program commenced on the Fraser Coast on April 12, 2021 and The PHN contracted Community Nursing In-Home Falls Assessment Program will compliment and support the wider Fraser Coast Falls Prevention Service referral pathway under the direction of the Wide Bay Hospital and Health Service Nurse Navigator.
