Assessment of digital solutions that identify individuals within a patient’s care management team

⁠⁠⁠Communication across multi-disciplinary, cross-organisational teams including carers and other informal care management team members is cann be impacted by a healthcare provider’s inability to easily identify who is in a patient’s care management team. This results in a decreased level of a patient’s quality of care, increased time taken for providers to discover information and increased time taken to communicate between the care management team.
Research and analyse existing software solution products nationally and internationally that provide the ability to identify the individuals within a patient’s care management team, and what key capabilities these include – e.g. How consent/approval is managed, searching and the user interface styles of different solutions
Undertake desktop research and analysis to identify evidence of current best practice with respect to use and management of the care management team
Produce a report of the outcomes and potential lessons learned from existing software solutions; this includes but is not limited to:
• Specific user requirements of the system
• Identify key attributes that are critical to the core functionality of the solution
• Define searching capabilities
• Document key use cases and user scenarios
• Comparison table of relevant features of the software or other digital products
