Tender Description
Expressions of interest are invited from General Practice membership to actively participate on the Statewide Older Person’s Health Clinical Network (SOPHCN) Steering Committee until the end of the 2021 financial year. The purpose of the SOPHCN is to:
- Provide expertise, direction and advice to the Minister of Health, the Director-General of the Department of Health, Deputy Director-General of the Clinical Excellence Division, Hospital and Health Services (HHSs) and clinicians on matters relating to the delivery of Older Person’s health care and evidence based best practice in Queensland.
The SOPHCN target client / patient group are those considered to be:
- Frail aged
- Healthy aged with respect to primary prevention
- Older persons across the continuum of health care
- Older persons in rehabilitation, and
- Older persons in subacute and long-term residential care.
