Expression of Interest – In Reach Primary Health Care Services at Early Morning Centre

⁠⁠⁠Capital Health Network (CHN) is seeking expressions of interest from interested parties for the provision of primary health care clinicians (General Practitioner or Nurse Practitioner) to work at the Early Morning Centre (EMC) clinic for two hours per week from mid-November 2020 to 30 June 2022.

The Early Morning Centre Clinical In-Reach program operates from the hours of 9-11am every Wednesday morning (excluding public holidays).

The Clinic services between 2 to 10 clients in the 2-hour window with an average of 5 occasions of service each Wednesday throughout the year. The clinic offers an irreplaceable service to a vulnerable population that would not be accessing services otherwise. Key interventions include support for diabetes, hypertension, wound care and crucial mental health linkages for those with anxiety, depression and schizophrenia.
