Supporting People with Severe Mental Health Conditions

⁠⁠⁠Darling Downs and West Moreton PHN is required to commission primary mental health care services for people with severe mental health conditions who are being supported in primary care. These services will include the provision of clinical care coordination which addresses both mental health and physical health needs as well as psychosocial support services to ensure an integrated approach to service delivery.

Clinical care coordination has been provided through the provision of mental health nurse care across the PHN region since 2018. Psychosocial support has been provided through a lived experience peer workforce under the National Psychosocial Support Measure from 2019. Both services have been working closely together to ensure clinical care coordination and non-clinical supports are closely integrated for people living with a severe mental health condition.

In this tender, the PHN is requesting submissions for Mental Health Nurse Care, Psychosocial Support services and CoS to provide a service that delivers integrated clinical and non-clinical services for people with severe mental health conditions.
