North Western Melbourne Primary Health Network (NWMPHN) is seeking to commission a provider to deliver a multidisciplinary self-management approach for individuals who suffer from chronic pain in the Northern Melbourne Region (Darebin, Hume, Moreland, Yarra, Melbourne, Moonee Ponds, Macedon Ranges).

NWMPHN is an independent, locally governed and run, not for profit organisation dedicated to improving primary health care in local communities.

NWMPHN is seeking to commission a multidisciplinary self-management program for individuals who suffer from chronic pain to improve their physical and mental wellbeing and enable them to self-manage their chronic pain.
